The Company operates through two segments: Home & Community Services and Episodes of Care Services. The Home & Community Services segment offers a variety of solutions to help manage the health of its customers’ members in their homes, including in-home health evaluations and related services, telehealth through virtual in-home health evaluations (IHEs), social determinants of health and biopharmaceutical services. Episodes of Care Services segment manages episode-based payment programs. It offers a variety of solutions to help its customers participate in episode-based programs, including Medicare Bundled Payment for Care Improvement (BPCI and BPCI-A), Commercial Episodes of Care, Transition to Home (TTH), Complex Care Management and accountable care organizations (ACO) Services.
